Accurate emergency information must be on file with the school. Please download and complete the LAUSD Student Emergency Information Form. Please keep this information up to date and inform the school if there are any changes. It is very important that you list more than one emergency telephone contact. Remember, the school can only release your child to adults listed on the emergency card.
